Do I put the amount of the doctor's bill or the copay in small claims court?
Also do I put the doctor's visit - $325 or the copay $20 ? I don't have a deductible.

You list the bill amount after adjustments for insurance. You do not just put co-pays. And understand that your health insurance almost surely has a lien on your settlement and you will have to settle that with the health insurance. So make sure your case includes the actual billed medical amounts.
