Why would a workman’s comp attorney deter you from surgery if you need it?

I would first suggest setting a face to face meeting with the lawyer either in person or over Zoom and express his dissatisfaction with his responses. I would have him explain what he is looking for and if this attorney legally cant make it happen he is free to find alternative counsel with no additional fee. In order to receive the kind of treatment that is thought to be needed he would need to have a medical opinion indicating that the treatment was in fact needed and causally related to the MVA in order to force WC to provide that medical care. It also appears that your family member has been out of work for an extended time and if that is the case he may also be able to apply for Social Security Disability benefits by contacting 800-772-1213. I hope this information is helpful.
This answer does not establish the existence of an attorney-client relationship nor is there any guarantee this advice will be successful in a court of law. A meeting to discuss your specific facts and a review of court orders and other relevant documents is needed for proper guidance and advice.
